Apply for Assistance

Families inquiring about assistance from the Mighty Mack Foundation should fill out our Assistance Request Form. Responders will be contacted regarding their approval. Please note that the type of assistance we are able to give changes as our foundation receives different grants, donations, and other support and depending on the amount of requests we receive at a certain time. It is our goal to be able to help every family to the best of our ability!
